COLLEGE OF ART AND ARCHITECTURE 

PETITION FOR REINSTATEMENT AFTER ACADEMIC DISQUALIFICATION AND/OR FINANCIAL AID SUSPENSION

If you are petitioning for reinstatement to the university and/or reinstatement of your financial aid, you must submit a typed letter prior to the beginning of the semester for which you are seeking reinstatement.  Your letter must include the following: Name, Student I.D. No., Local Address, Local Phone, E-Mail Address, your Major, and answers to the questions below.  Provide as much information as possible that you think will help support your case. 

 QUESTIONS PERTAINING TO ACADEMIC DISQUALIFICATION

*Please give as much information as possible with emphasis on questions 2 and 3*
  1. What are your academic goals?

  2. List honest, specific reasons why you did not do well academically last semester (or when you were enrolled) especially when you knew you were on academic probation.

  3. List the classes you plan to repeat and the specific changes that you would make to improve your performance this next semester if you were reinstated.

  4. List campus resources that you would use to help reach your target academic goals.

We recommend you attach all documentation that will support your case.
Examples might be:

  1. Letter from your academic advisor.

  2. Letter from counselor or psychologist.

  3. Letter from physician if chronic health problems contributed to your academic difficulties.

 QUESTIONS PERTAINING TO FINANCIAL AID SUSPENSION 

  1. Why did you fail to meet the minimum GPA requirements and/or minimum number of credits required by the Financial Aid Office?  Give specific reasons for your poor academic performance if that is the case.

  2. If you withdrew from a semester for medical or personal reasons, please provide an explanation and provide medical documentation.  The Committee will want to know if your medical problems have improved since the time of your withdrawal from the University.

  3. Why is the College justified in recommending the reinstatement of your financial aid?  What will you do to guarantee that you will meet the required GPA and progress from this point at a reasonable rate of at least 12 credits per semester if you receive full financial aid?
